Resources for Integrated Care (RIC) invites you to attend a webinar featuring speakers from health plans that serve people eligible for both Medicare and Medicaid. These speakers will share strategies related to engaging hard-to-reach members during the COVID-19 pandemic.
As a population, people eligible for both Medicare and Medicaid are more likely to have one or more chronic medical conditions and serious mental illness (SMI) than Medicare-only beneficiaries. They also experience more negative social drivers of health. The COVID-19 pandemic has made in-person engagement much more difficult and has magnified health and social needs. During this time, reaching these members is both more pressing and more challenging.
In 2021, RIC launched Integrated Care Community of Practice (ICCoP), a learning community of outreach and care coordination staff from 20 national and local health plans focused on sharing promising practices for reaching and engaging members in primary and specialist care, social services, and long-term services and supports.
This panel discussion will offer promising practices that emerged from the ICCoP and elsewhere. By the end of this discussion, attendees should be able to identify strategies for:
- Locating hard-to-reach members, including through the deployment of community health workers and through partnerships with community-based organizations.
- Addressing unmet social needs and building trust through tangible supports for members.
- Developing information system approaches to contacting hard-to-reach members.
- Reaching members with limited English proficiency, those experiencing homelessness, or those with SMI.
- Providing COVID-19 vaccine outreach and education to hard-to-reach members.
