Join the Partnership to Align Social Care for a webinar about the 2024 Medicare Physician Schedule Proposed Rule. The annual proposed rule detailing calendar year 2024 anticipated payment adjustments under the Physician Fee Schedule and other Medicare Part B policy updates includes several important changes. There will be an overview and discussion of these changes, as well as opportunities to inform the public comment process.



The Centers for Medicare & Medicaid Services (CMS) is proposing coding and payment changes for services to address health-related social needs through separate payments for community health integration (CHI), social determinants of health (SDOH) risk assessment, and principal illness navigation (PIN) services. The proposed codes are the first that are specifically designed to describe services involving community health workers, care navigators, and peer support specialists, allowing these services to be provided by personnel employed by community-based organizations (CBOs) under the general supervision of the billing practitioner.



CHI and PIN services involve a person-centered assessment to better understand the patient’s life story, coordinate care, contextualize health education, build patient self-advocacy skills, navigate the health system, facilitate behavioral change, provide social and emotional support, and facilitate access to community-based social services to address unmet SDOH needs.



Also included are changes to the payment mechanism for SDOH risk assessment activities — including allowing assessment as an optional additional element with additional payment during the annual wellness visit. Other potentially significant updates for evaluation and management visits, telehealth services, outpatient diabetes self-management programs, and behavioral health services are included in the proposed rule.



This event is intended for community-based organizations, community care hubs, health plans, health systems, provider stakeholders, and other advocates who want to learn more about what proposed coding and payment changes could mean for addressing health-related social needs and the overall effort to align health and social care ecosystems.
