Session 1: Emerging Findings on Strengthening Housing, Health, and Social Care Partnerships
On September 18, 2024, HSRC hosted a webinar on emerging findings on strengthening housing, health, and social care partnerships. A growing number of states are leveraging new opportunities under Medicaid to cover services and supports that help people find, obtain, and maintain their housing. In 2024, the Housing and Services Resource Center (HSRC) launched the Housing and Services Partnership Accelerator (HSPA) to advance innovative state strategies to provide housing-related services and supports to Medicaid-eligible people with disabilities and older adults who are at risk of or presently experiencing homelessness. HSPA has helped eight states and the District of Columbia strengthen collaboration across Medicaid, housing, aging and disability (social care), and behavioral health systems resources and programs.
A new three-part webinar series highlights state innovations and cross-sector collaborations to improve the coordination of housing assistance and wrap-around services and supports as a cost-effective approach to assisting people with disabilities and older adults to transition from homelessness, exit or avoid institutional settings, and live stably in the community.
Session 1: HSPA National Webinar
Select HSPA states will share best practices, challenges, and lessons learned as they develop and implement effective partnerships and innovative solutions to advance housing stability. Presenters will briefly review federal collaborative efforts and describe the individualized technical assistance and facilitated peer learning opportunities offered to states through the HSPA. Participants will learn about the growing array of HSPA and related HSRC resources to support state and local efforts to align housing, health, and social care.
