Join the Disability Rights and Education Defense Fund (DREDF) and the Aging and Disability Partnership for Managed Long Term Services and Supports for the first in a series of webinars examining promising practices in collaborations among Independent Living Centers, Medicaid managed care plans, Area Agencies on Aging, Aging and Disability Resource Centers, and clinical settings. The goal of these collaborations is to re-envision the independent living philosophy as a central element of both care coordination and long-term services and supports (LTSS).

The webinar on Thursday, October 24 highlights the work of the Aging and Disability Resource Connection, a collaboration between the Independent Living Center—FREED, N4AA, and other LTSS and safety net providers in rural California. The collaboration helps individuals with disabilities plan for LTSS; provides care transition intervention for people with substance use disorders and mental health disabilities; and facilitates community transitions.
