ACL Announces Participants Selected for Next Community Care Hub National Learning Community

ACL is pleased to announce the selection of 32 organizations for participation in the 2023-2024 Community Care Hub National Learning Community. The organizations, which have representation across 23 states, will engage in shared learning, information and resource sharing, and technical assistance coordination with the goal of building the strength and preparedness of each community care hub (CCH) to address health-related social needs (HRSNs) through contracts with health care entities. 



The National Learning Community (NLC) will build on the curriculum and other technical assistance of the 2022-2023 cohort, including use of the ECHO Model® “all teach, all learn” approach and individual consultation with subject matter experts. The curriculum will be organized into modules focused on the following topics:

  • Health care contracting opportunities
  • Creating CCH value propositions
  • Contract negotiations

CCHs play an important role in promoting an equitable health and social care ecosystem. They serve as the bridge between community-based organizations, health care payers and providers, and public health systems to increase a community’s capacity to reach populations who are underserved. They achieve their goals through coordination and provision of services that address HRSNs. 



Building the capacity of CCHs nationwide advances ACL’s vision of coordinated person-centered care and services for all community members. The U.S. Department of Health and Human Services has also emphasized the important role CCHs play in addressing HRSNs through a Call to Action, highlighting the roles and actions community-based organizations, health plans, health care providers, and public health and other stakeholders can take to support these community partnerships across the nation. The NLC is one of several actions ACL is taking to support the development of CCHs and sustainable community partnerships nationwide.
