Apply for the CoP
Commit to Connect (CTC), an ACL initiative, invites applications from aging and disability network organizations for a community of practice (CoP) focused on measuring the social connection impact of chronic disease self-management education (CDSME) programs through pre- and post-test measures. CoP participants will meet five times between March and September 2026. Members will receive no-cost access to collection instruments, technical assistance, and support from CTC staff and a leading researcher, Matthew Lee Smith, PhD.
The CoP is part of CTC’s aim to increase the availability of evidence-based programs and services that address social isolation and loneliness.
