ACL is releasing a new funding opportunity to help states measure and quantify the impact of their No Wrong Door (NWD) systems. Funds will specifically be used to identify and evaluate data that demonstrates how person-centered counseling and evidence-informed interventions, such as care transitions, nursing home diversion programs, and the Veteran-Directed Home and Community Based Services (VD-HCBS) program, empower individuals to access quality long-term services and supports in ways that meet their needs and delay or prevent the use of more costly services.
Additional project goals include working collaboratively with ACL and its partners to develop a methodology and toolkit for calculating a return on investment (ROI) for NWD systems. Grantees will be expected to share key data elements with ACL to test and calculate ROI at a national level and produce a business case model that demonstrates the impact of NWD systems.
Grants up to $600,000 each budget year will be awarded to up to nine states for a 24 month project period. An informational call and webinar for interested applicants will be held on April 25, 2018 at 2:00 pm, ET.
