2022 Empowering Communities to Deliver and Sustain Evidence-Based CDSME Programs

The intent of this funding opportunity is to help communities develop or expand capacity for, deliver, and sustain evidence-based CDSME programs for older adults and adults with disabilities, particularly those in underserved geographic areas and populations.

2022 Empowering Communities to Deliver and Sustain Evidence-Based Falls Prevention Programs

The intent of this funding opportunity is to help communities develop, deliver, and sustain evidence-based falls prevention programs that reduce falls and falls risk among older adults and adults with disabilities, particularly those in underserved geographic areas and populations.
