Home modifications that increase safety and support in the home environment can promote independence and community participation. Effective home modification service delivery involves professionals and funding sources from the aging, disability, housing, and health care sectors. This Housing and Services Resource Center webinar offers an overview of types of partners to engage in cross-sector collaboration for expanding access to home modification. Presenters share first-hand experiences with establishing partnerships across the housing, aging, and disability sectors, including assistive technology. They discuss how collaboration has helped them to meet the home modification needs of people with disabilities and older adults. Resources for educating consumers and identifying home modification services and funding sources in each state are provided.
The Housing and Services Resource Center was created for people who are working to help people live in the community by fostering collaboration and cross-sector partnerships to streamline access to services and make community living possible for more people. The Center is part of a partnership between the U.S. Department of Health and Human Services and the U.S. Department of Housing and Urban Development to make community living a reality for all. This partnership will expand accessible, affordable housing; help people exit homelessness; improve home and community-based services; and address the institutional bias in America's long-term care system.
