Chronic Disease Self-Management
Good nutrition is an important part of managing chronic health conditions and supporting healthy aging. Evidence-based Chronic Disease Self-Management Education (CDSME) programs complement nutrition and other health promotion efforts by helping older adults develop the knowledge, skills, and confidence to manage their conditions, adopt healthy behaviors, maintain independence, and improve quality of life.
