The Better Living through Education and Nutrition for persons with Diabetes (BLEND)
This pilot project aimed to drive healthcare investments in Older Americans Act (OAA) services by implementing a Food is Medicine intervention. It integrated Title III-C home-delivered meal services and Title III-D diabetes programs to improve health outcomes in several counties in central North Carolina. In collaboration with a regional Area Agency on Aging (AAA), the initiative connected older adults with diabetes to medically tailored meals and Diabetes Self-Management Education and Support (DSMES).
Addressing Ability to Eat with Modified Meals
To build the evidence needed to improve the effectiveness of home delivered meals, Live ON NY partnered with several organizations to provide therapeutic meals modified to align with dental and oral function and promote oral health and dental care.
WeCare Services Personal Health Nursing for Seniors Receiving Title III Home-Delivered Meals
Education, Health and Research International utilized the Title III Senior Nutrition Program organization’s established relationships with homebound individuals to identify those who would most benefit from personalized, no-cost, nursing services. WeCare Personal Health Nurses established one-on-one client relationships that are marked by routine communication, practical problem-solving, liaison to medical services and benefits and linkages to available community resources.
Offering Nutrition-Focused Diabetes Self-Management
To address patient care beyond the clinic, Health Promotion Council of Southeastern Pennsylvania, Inc. offered a multi-component, home-based intervention to improve health outcomes at the patient level, and reduce health care costs at the system level, for older adults with Type 2 Diabetes.
SixtyPLUS: How Meals-on-Wheels Redefines Population Health
LifeCare Alliance, in collaboration with partners, concentrated on three key innovations designed to improve health outcomes. These included building and formalizing partnerships with local fire and EMS departments, combining Registered Dietitian consultation services with home-delivered meals, and implementing the Home Care Frailty Scale to assess changes across a wide range of functional and health domains.
