Providing peer connection, wellness education, individual nutrition support, and Medically Tailored Meals to socially isolated older adults in urban and rural areas. Utilizing telehealth, peer groups, and individualized support to promote nutrition security and build confidence in managing health.
A Dietitian with a Side of GRITS: Implementing Virtual Nutrition Education Across Georgia
Open Hand Atlanta developed innovative solutions to bridge the gap between health and nutrition through a range of partnerships. Older adults at high nutritional risk were connected with registered dietitian nutritionists for medical nutrition therapy support. Additionally, virtual monthly nutrition sessions that were offered to senior centers to provide consistent, evidence-based nutrition education.
Improving Nutrition and Health of Rural Older Adults Following Hospital Discharge
Eastern Area Agency on Aging assisted older adults with chronic conditions during their transition from hospital to home. The project offered nutrition interventions that included meal delivery and nutrition counseling, aiming to enhance outcomes related to nutrition, chronic disease self-efficacy, and to reduce emergency room visits and hospitalizations.

Interfaith Ministries for Greater Houston investigated the ways Amazon Alexa technology can support the social determinants of health together with Meals on Wheels deliveries to improve health outcomes, reduce isolation, and provide innovations to support the principles of aging in place.
Improving Client Service Through Advanced Data-Card Technology
To improve service delivery and cost-effectiveness of nutrition programs for older adults at risk of malnutrition, the MidAmerica Regional Council and Aging Ahead Area Agency on Aging deployed artificial intelligence-enabled speakers, Amazon Echo Show, to clients' homes to reduce access barriers to good nutrition.
Modernizing Home-Delivered Meals Through Frozen Meals and a Web-Based App
To modernize the home-delivered meal program, Eskenazi Health’s Meals at Home in partnership with other organizations, develop an app for older adults at risk of food insecurity and malnutrition. This app allowed participants to order frozen meals and gauged participant satisfaction before, during, and after the process.
Increasing Use of Community-Based Resources by Food-Insecure Seniors
To reduce food insecurity, Age Options designed a Closed Loop Referral System project to ensure that patients facing food insecurity or nutritional risk received and acted on referrals to community resources. This system enhanced access to nutrition programs, government benefits, and evidence-based health and wellness initiatives.
