Profiles for ACL’s 2017 Chronic Disease Self-Management Education (CDSME) grantees are now available on our website. These eight organizations have proposed to collectively reach 38,000 older adults and adults with disabilities through evidence-based self-management programs, empowering them to better manage their chronic conditions.
The organizations who have been awarded the 2017 Prevention and Public Health Fund CDSME grants are:
