Chronic Disease Self-Management Education (CDSME)

• Integrated Networks to Deliver and Sustain Evidence-Based CDSME Programs (HHS-2021-ACL-AOA-CSSG-0014). This opportunity aims to strengthen networks that address the behavioral and social determinants of health of older adults and adults with disabilities, and significantly increase participation in CDSME and support programs.
• Empowering Communities to Address Behavioral Health and Chronic Pain through CDSME Programs (HHS-2021-ACL-AOA-CSSG-0018). This opportunity aims to address behavioral health and/or chronic pain among older adults and adults with disabilities through the implementation of appropriate evidence-based education and support programs.

Evidence-Based Falls Prevention

This opportunity aims to strengthen networks that address the behavioral and social determinants of health of older adults and adults with disabilities, and increase participation in evidence-based community programs to reduce falls and falls risk.
• Empowering Communities to Reduce Falls and Falls Risk (HHS-2021-ACL-AOA-FPSG-0015). This opportunity aims to help communities reduce falls and/or falls risk among older adults and adults with disabilities through the implementation of evidence-based programs.
