ACL provides community inclusion, integration, and access programs to support community living opportunities for older adults and people with disabilities. These activities focus on expanding accessible transportation, enhancing care transitions, promoting assistive technology, and other areas.
- Transportation - ACL works with communities across the nation to give travelers a voice in the planning process and to improve transportation options.
- Care Transitions - This effort supports states to strengthen the Aging and Disability Resource Centers in implementing evidence-based care transition models to engage older adults, people with disabilities, and their caregivers for improving care transitions.
- Employment and Youth Transitions - ACL supports 'employment first' initiatives for people with disabilities and programs to help youth with disabilities transition to adulthood.
- Veteran Directed Home & Community Based Services - This program provides veterans with opportunities to self-direct their long-term services and supports and continue living independently at home.
- Help America Vote Act Programs - These programs are design to establish and improve participation in the election process for individuals with the full range of disabilities.
- Assistive Technology - These programs support making assistive technology devices and services more available and accessible to individuals with disabilities and their families.
- Senior Centers and Supportive Services for Older Adults - This program provides grants to states and territories that fund multi-purpose senior centers that coordinate services for older adults, such as congregate meals, community education, health screening, exercise and health promotion programs, and transportation.
