It will include a description of a collaborative care coordination model used in Virginia to help individuals living with dementia. In particular, the focus will be the challenges experienced by people living alone with dementia and challenges related to reaching them.
Complimentary CEUs available through this free webinar. Closed captioning is available. The webinar will be recorded and available to view at nadrc.acl.gov. If you have questions, please contact Sari Shuman.
