Join this webinar to learn how two organizations have engaged CHWs in implementing evidence-based chronic disease self-management education and falls prevention programs. Speakers will discuss a chronic care management program that has been jointly developed and implemented by a federally qualified health center and university, as well as a community health worker online training program focused on falls prevention and chronic disease self-management.
