HHS' Assistant Secretary for Preparedness and Response recently published Discharge Planning and Care Coordination during the COVID-19 Pandemic, a resource developed in partnership with ACL and the Centers for Medicare & Medicaid Services.
This tool is designed to support nurses, social workers, case managers, and others conducting discharge planning for adults with disabilities after COVID-19 treatment.

ACL awarded one cooperative agreement to the ARCH National Respite Network and Resource Center to operate the Lifespan Respite Care Program: Technical Assistance and Resource Center (TARC). Transitions, including from school to college or work in the community and from pediatric to adult health care providers, can present unique challenges for people with Intellectual and developmental disabilities (IDD). One significant challenge is that the transition services and supports currently offered to youth and adults with IDD by school and health systems are often fragmented and difficult to navigate.
