The 15th Anniversary of the groundbreaking Supreme Court decision in Olmstead v. L.C. is coming up this June. To commemorate the anniversary, the Department of Justice (DOJ) is planning to develop a short compilation video highlighting individuals with disabilities who have benefited from Olmstead. DOJ is seeking video profiles for use in the compilation video. Instructions for submitting a video are as follows:

DOJ will review all videos received by March 31, 2014, and will include portions of some of the videos we receive in the final video compilation.

Submit a signed consent form attached as separate document (see below).

Send your video on a USB drive, labeled clearly with your name, city, state, and contact number (optional) to the following address:
