Join the ACL National Falls Prevention and CDSME Resource Centers (operated by the National Council on Aging) for a four-part webinar series that aims to raise awareness about the role of Community Health Workers (CHWs) in the implementation of evidence-based programs, discuss avenues for sustaining and reimbursing CHWs, and highlight how CHWs can help organizations reach diverse populations and address social determinants of health factors.
Community Health Workers (other titles include peer health educators, lay health advocates, community health representatives, promotoras, or community resource coordinators) serve as a connection between medical and social services and their communities.

The webinar series include four live 60-minute webinars on February 22, March 9, 16, and 24.
