Social Care Referrals Challenge Outreach Toolkit

We need your help promoting ACL’s Social Care Referrals Challenge to create and modernize health IT solutions to foster connections between health care systems and community-based organizations that provide social care for older adults and people with disabilities.

We ask you to spread the word about a technology development opportunity with cash prizes totaling $500,000. Specifically, we need help reaching health IT developers, health insurers, and large healthcare providers who may want to partner with community-based organizations to participate in this Challenge.

This toolkit contains materials and messaging that you can use to promote the Social Care Referrals Challenge. The toolkit includes an email template, Facebook and Twitter posts, recommended hashtags, and graphics for promotions. Feel free to customize your messages and send them to your contacts. The deadline to register and submit for Phase 1 of this Challenge is December 14, 2020.

Subject/Title: Apply for the Social Care Referrals Challenge to modernize health IT platforms for connecting healthcare systems with social care services

Social care services delivered in homes and communities prevent falls, address food insecurity, provide transportation for health appointments, manage chronic diseases, and address many non-medical risk factors. Addressing these social needs has been shown to improve health outcomes and reduce the cost of healthcare.

Community-based organizations have been a long-trusted resource for meeting these kinds of needs and delivering services. They have unmatched expertise in addressing local needs, service coordination and delivery, and supporting the independence and well-being of older adults and people with disabilities. Partnerships with community-based social services organizations have proven to be an effective way for healthcare organizations to address social determinants of health.

Partnerships between healthcare organizations and community-based organizations are growing in both number and importance, along with the need for interoperable IT infrastructure. The Administration for Community Living at the U.S. Department of Health and Human Services has announced a Challenge competition that will award up to $500,000 in prizes for developing or optimizing interoperable and scalable technology platforms for healthcare organizations and community-based organizations to share data securely. This Challenge includes developing prototypes and implementing technical solutions that track referral patterns, gaps in service, and outcomes. Submissions are expected from a team of stakeholders with strong preference given to teams that include state leaders across aging, disability, and Medicaid programs; community-based organizations in the aging and disability networks; health IT developers; healthcare providers; health plans; and others with expertise in technical standards, interoperability, and data analysis. The Challenge will run in three phases, with cash prizes awarded in each, totaling $500,000.

The deadline for Phase 1 registration and concept & design submission is December 14, 2020.
